Assumption of Risk, Waiver of Liability, and Indemnification.

I understand and agree that the Program(s) I have purchased with this Agreement involves the risk of injury. I elect to participate in the Program(s) voluntarily despite the risk. I further understand and agree that the terms of my 

 

General Terms Agreement and Member Usage Agreement continue to apply, including the assumption of risk, waiver of liability, and indemnification provisions contained therein.

 

Cancellation by SK Basketball Policy. If an SK Basketball trainer cancels a class, a refund or a makeup session will be granted for any individual or group sessions.

 

Cancellation by Individual or Group Policy.  If I, or my group, am unable to attend a scheduled session, I and/or my group will contact my designated SK Basketball trainer at least 24 hours before the scheduled session. I will forfeit, and I agree to pay for, any sessions I fail to attend without such 24-hour advance notice and any sessions that I do not use within the month, given with grace of one cancellation per month, in which I purchased such sessions.

 

Cancellation for Medical Reasons.  For medical disabilities, a prorated refund or credit will be given when the participant provides a doctor’s note stating the participant can no longer participate in the program.

 

Personal Information and E-mail.  I understand that by providing my email address, I consent to receive related and commercial email messages from SK Basketball Academy. 

 

Release of Image and Likeness.  I  irrevocably consent to and grant SK Basketball Academy the exclusive and unlimited right to use and reproduce any and all photographs, audio recordings, video recordings, or testimonial accounts taken by SK Basketball Academy that contains my person, name, image, voice, likeness, for any lawful purpose whatsoever and using any means available including, but not limited to, any SK Basketball Academy or marketing communication or material. I further understand that SK Basketball Academy posts to SKbball.com and other social media networks and any posts will be visible to the public. I waive the right to inspect, approve or edit any such use or reproduction, and SK Basketball Academy may make any and all changes, modifications, rearrangements, additions or deletions in its use or reproductions without any approval.

I have read this Agreement thoroughly, understand all of its terms, including the Cancellation Policy, received a copy if I request it, and have knowingly and voluntarily signed it.
